The Vaccine Assessment Grid (“Grid”) is the document detailing the dosage-based assessment amounts for vaccines administered by providers and covered by the state’s Childhood Vaccine Program. **NEW** This short video describes essential information about the Grid.
Please update your fee schedules for dates of service effective July 1, 2026.
The current Grid is in effect between July 1, 2026 and June 30, 2027.
NOTE: ALL COVID-19 and one Influenza Grid amounts are increasing effective July 1, 2026. There is one NDC change for RSV ABRYSVO.
