WVA Private Insurance Assessment Billing Provider Guide

Submitting dosage-based assessments (DBA) with WVA is easy. The WVA Private Insurance Assessment Billing Provider Guide is a PDF which contains the information required to be compliant with the WVA’s governing statute and Plan of Operation in an easy to follow brochure.

If your provider practice is unsure whether a DBA is required, please refer to the State of Washington Department of Health’s (DOH) Eligibility for Publicly Funded Vaccines: A Guide for Providers available in PDF form. This document is also found on the WA DOH’s Patient Eligibility web page.

Online Video Tutorial:

In partnership with the WA DOH, there is a video tutorial that explains the WA DOH eligibility screening process and the WVA DBA process.

For Hospitals:

A Hospital Billing Workgroup was established in late 2023, comprised of hospitals, payers, WA DOH staff, and WVA staff. The Workgroup developed the WVA Hospital Inpatient Billing Guide intended for hospitals and payers to ensure their respective billing and adjudication systems can support the DBA process no later than July 1, 2024.
